Records


We take patient confidentiality seriously and all personal information is treated in strict confidence and records are stored securely.  We ask you to complete a medical history questionnaire to assist our diagnosis and treatment planning.  Patient information is confidential and dental records will only be released to third parties if written permission is given by the patient (or a relative or guardian,  in special circumstances).  This service may incur a copying fee. If you require more information please ask any member of our team.

Complaints
